⚕️ Side Effect Guidance: Include the medication name, specific symptom, when it started, and duration. Side effects from Donepezil, Memantine, Seroquel, and Depakote can overlap with dementia symptoms and may be expected and temporary. Always report to the supervising nurse or physician. Never adjust or discontinue any medication without physician guidance.

📋 Incident Report
Falls, injuries, medication errors, and events requiring formal documentation
📋 What to log here

Use this section for specific events that need to be formally reported — a fall, a near-fall, an injury, a medication error, a choking episode, or any behavioral crisis that required calling a physician or family member. These are events where something happened that someone else needs to know about officially.

Examples: Found on floor · Refused all medications · Struck caregiver · Elopement attempt · Injury during personal care · Sudden change in condition. For repeated behaviors and mood patterns — agitation, restlessness, refusal of care — use the Behavior Patterns section instead.

ℹ️ About Medication Warnings

A Medication Warning (also called an FDA boxed warning) is the FDA's most serious drug safety alert. It means the medication has a known risk of serious or life-threatening side effects. As a caregiver, your role is to watch for the listed symptoms and report them immediately to your doctor or pharmacist — never adjust or stop medication without medical guidance.

🌅 Sundowning Tracker
Document late-day confusion, agitation, and behavioral changes
🌅 About Sundowning — Important Guidance

Late-day confusion and agitation in dementia patients is often called "sundowning." Document every episode — patterns across multiple entries reveal far more than a single log.

⚠️ Sundowning can mask other causes. Before attributing behavioral changes to sundowning, consider whether the patient may have an undiagnosed UTI, untreated pain, constipation, dehydration, a medication timing issue, or overstimulation. These causes are treatable — sundowning progression is not.

Document: exact time, behaviors observed, possible triggers, what helped, and how long the episode lasted.

Call the physician if: the episode is significantly worse than usual, the patient has a fever, shows signs of pain, has not urinated in 8+ hours, or the behavior change is sudden rather than gradual. Sudden change is a medical signal, not a dementia symptom.

🚶 Wandering & Unsupervised Exits
Document wandering incidents and unsupervised exit attempts
⚠️ Wandering & Unsupervised Exits — Safety Guidance

Document every incident immediately. A wandering log is only clinically useful when entries are consistent and complete. Time of incident, duration, location found, and what the patient was doing are all essential.

Wandering vs. Leaving Unsupervised: Wandering is movement within a supervised space. Leaving unsupervised — clinically called elopement — means the patient has left or attempted to leave without authorization. This is a medical emergency. If a patient leaves or attempts to leave, call 911 immediately, then notify the family and supervisor.

Before logging, verify safeguards are in place: door alarms active, bed sensor functioning, ID bracelet on patient, family notified of risk level.

Call the physician if: wandering episodes are new or suddenly increasing in frequency, the patient appears in pain or distressed during episodes, or the patient is found in an unsafe location. A sudden increase in wandering behavior can signal a UTI, pain, medication reaction, or disease progression.

💾 Data Management
📤 Back Up Your Data
⚠️ Important: All your data is stored on this device only. If you clear your browser or app data, or switch phones, your data will be permanently lost. Export a backup regularly.

Tapping Export All Data saves a complete backup of everything in the app — all patients, medications, medication log records, incidents, care plans, wounds, behaviors, billing, vitals, pain logs, and credentials — as a single .json file on your device.

What to do with the file:
• Save it to Google Drive or your phone's Downloads folder
• Email it to yourself as a personal backup
• Save it before switching phones or reinstalling the app
⏰ Recommended: Export at least once a week, or after every shift if documenting critical care.
📥 Restore from Backup

Tapping Import Backup lets you select a previously exported .json file and restores all your data — care recipients, records, and settings — exactly as they were when you exported.

When to use it:
• You got a new phone and need to transfer your data
• You reinstalled the app and lost your records
• You accidentally cleared the app's storage
• You want to restore an older backup
⚠️ Important: Importing a backup will merge with your existing data. New entries in the backup file will be added — existing records will not be deleted. Always export your current data first before importing an older backup.
⚠️ Compliance Notice: Ensure device security meets your agency's HIPAA policies. This app stores data locally only — no cloud servers.

🔒 How Your Data Works

Your care records are stored privately on this device — no cloud servers, no third party access, no subscription required to keep your data.

Because data lives on this device it is not automatically synced to other phones or tablets. If multiple family members share caregiving responsibilities, designate one shared device as your caregiving hub so everyone works from the same record.

🧠 Behavior Patterns
Track what triggers behaviors and what helps — builds patterns over time
🧠 What to log here

Use this section to document repeated or pattern-based behaviors — agitation, restlessness, refusal of care, verbal outbursts, mood changes, or sundowning episodes. The ABC format captures three things: what happened before the behavior (the trigger), what the behavior looked like, and what helped resolve it.

Why it matters: A single behavior entry tells you little. Ten entries showing agitation occurs every afternoon when visitors arrive tells you everything. The pattern is where the clinical value lives. Use the Incident Report section instead for one-time events like falls, injuries, or medication errors that need to be formally reported.

🧠

😣 Pain Scale Log
Structured pain assessment — 0 to 10 with face scale
😣 About This Pain Scale — Important Guidance

The 0–10 face scale is for patients who can communicate their pain verbally or by pointing. For patients with moderate to advanced dementia who cannot self-report, this scale alone is not reliable. A patient who scores 0 may still be in significant pain — they simply cannot express it.

For non-verbal dementia patients, observe behavior instead of asking. Signs of unrecognized pain include: facial grimacing, bracing or guarding a body part, moaning or calling out during movement, resisting care, increased agitation or confusion, changes in breathing, or refusing to bear weight.

When documenting pain: Record what you observed, not what you assumed. "They grimaced and pulled their arm away during dressing" is more useful than "pain score 4." Specificity helps the physician make better decisions.

Call the physician if: pain appears sudden or severe, the patient shows signs of pain at rest (not only during care), pain seems to come from a new location, or behavioral changes are unexplained. Pain in dementia patients is consistently underdiagnosed and undertreated.

How are you right now?
Not your loved one. Not the shift. You.
You showed up today.

For someone who may not always know your name. Who may have been afraid in the night. Who needs you in ways most people around you will never fully understand.

Tired but okay is more than enough. It is actually everything.
— Frank, fellow caregiver · tendedhealthapps@gmail.com
I hear you.

A heavy day does not mean you are failing. It means you have been giving more than you have for longer than anyone should have to.

That is not weakness. That is all of us.

Just look for it. Take your time. When you find it — tap the button below.
These are true. Right now. Even if you cannot feel them yet.

You did not have to show up today. You chose to. That choice matters more than you know.
The fact that you feel this heavily means you love this deeply. That is not a burden. That is who you are.
Done is enough today. You do not need to do this perfectly. You just need to get through it.
This moment will pass. You have gotten through every hard moment before this one. You will get through this one too.
You are not alone in this. There are people in this community awake right now carrying exactly what you are carrying.
I see you.

Not okay is a real place to be. You are not being dramatic. You are not weak. You have been holding something enormous for a very long time and right now it is too heavy.

Stay with this screen for a moment. You do not have to do anything yet.
